Key responsibilities Perform telephonic outreach to enroll and engage patients, serving as their primary point of contact Develop and implement individualized care plans in collaboration with the interdisciplinary team, facilitating seamless transitions of care Conduct thorough assessments to identify social determinants of health, connecting patients to necessary resources and advocating for their needs Required qualifications 3 years of experience in healthcare coordination or community health work Experience with a team-based approach to care Excellent knowledge of community resources relevant to patient care Strong ability to prioritize tasks and adapt to changing patient needs Intermediate skills with MS Office Suite, including Outlook and Teams
